If you're on the motorcycle and just sit there for a little while. Niko gets one from nowhere and puts it on. It's useless.

You can't really "get " (like find or buy) a helmet in GTA4. The only way you can put on a helmet is when you get on a bike, don't start moving right away. If you wait 1-2 seconds, Niko will get one out of thin air (off the bike presumably) and put it on. But if you start moving right when you get on the bike, he doesn't put on the helmet. The point is logical like in real life, if Niko crashes or flies off the bike, the damage will be less if he is wearing the helmet. So unless you're about to be caught by cops or someone trying to fight you, it's usually best to wait the 1-2 seconds to put on the helmet before riding. If you do have to ride off immediately, then stop at some safe place later on, get off the bike, then back on, and wait the seconds to get the helmet on.
